Cancellation Policy/ Consent Forms
I work with a 24 hr notice policy for appointments cancelled, postponed or rescheduled at short notice. This is flexible for emergency cancellations on both parties.
I work with a client consent form and will require you to sign this as part of any work that we do together.
Payment/Fees:
I request payment to be made prior to or on the day of each therapy session.
I also work within a sliding scale and this can be negotiated with you on an individual basis.
Parental Consent Forms:
I use a 'Parental Consent Form' when working with younger people who's parents have involvement in their therapy and involve the young person where Gillick Competent.
Young people have the right to their confidentiality being maintained unless there is a concern such as a Safe Guarding/Child Protection issue and you as the parent/guardian will be required to sign a specific form to agree to this and before any therapy is undertaken with a minor.
This will also be subject to regular reviews.
Covid Policy:
I have a written Covid Policy in place and kindly ask that this is respected to adhere to my Health and Safety within my practice and to ensure your Health and Safety whilst undergoing therapy
Online Counselling & Supervision:
I am fully qualified and registered and insured to deliver Counselling and Supervision remotely.
I ask that the same principle applies in regards to any fees being paid on the day where possible and have a separate contract in place for Online Therapy.
